Myth 3: “My filler will last forever, right?”

The honest answer to this question is NO they will not last FOREVER with only ONE treatment. Dermal fillers are fillers that treat volume loss in most areas of the face including lips, nasal labial folds, chin, cheeks, tear troughs, under eyes, jaw line and  temples.  Most hyaluronic acid fillers (Restylane, Perlane, Silk, and Juvederm) will last anywhere from 6-12 months with one treatment. However; if you can tolerate minor maintenance about every 6-8+ months, you can achieve and maintain amazing results that last.   Keep in mind results also depend on which filler you choose, what area we treat, and how much product we use. The most common filler complaints I have encountered are a result of inadequate amount of product being placed.  Perhaps you needed 2 syringes and chose to place only one syringe, you may not achieve your optimal result. For this very reason I am all about consultation and catering to each patient’s needs and what will work best for your lifestyle. Another hyaluronic option is Juvederm Voluma, specifically for cheek volume. With adequate treatment you can expect to see results for up to 2 years. The last option is Sculptra Aesthetic, or poly-L-lactic acid (PLLA). This is also a filler used to add volume; however, in a few days the initial volume diminishes, and the PLLA microparticles remain to stimulate your own collagen production. With adequate treatment you can expect to see natural results for 2-5 years. I strongly recommend you consult with your practitioner and ask plenty of questions prior to having a treatment. This will help to eliminate any confusion, and also aid in the selection of type and amount of product that best fits YOU.  🙂

If I get Pregnant and Breastfeed, what happens to my Breast Implants?

pregnant womanGreat question with an easy answer…absolutely nothing.

When we place breast implants, the normal method is to place the implant behind the muscle. So what is done is that the breast and muscle are simply lifted forward and the implant slots in behind. So, in the event of a pregnancy or breast feeding the breast acts exactly as it would with or without an implant in place. And after the pregnancy and/or breast feeding, the breast tissue will change as it would with or without an implant.

The implants themselves are inert, man made devices, they do not change in response to the changes in the overlying breast when pregnancy occurs. So, they are unaffected.

What can change is the perkiness of the breasts. If the breasts get extremely large and the skin stretches it can lead to a drooping problem. But again, this is a breast problem, not an implant problem. Also, there is absolutely no evidence that having a breast implant in place, saline or silicone filled, affects the breast milk either.

Hope this helps to answer a fairly common question particularly among our younger implant patients.

Swelling after Surgery

By far the most common questions I field in my Aesthetic Surgery practice following a surgery concern swelling. This is true for facial surgery, breast surgery and body contouring. “Is this normal ?” and “When will it go away ?” are usually the main ones. Well, it is completely normal for your tissues to swell after surgery and it will go away at different speeds depending what was done and the body part. Here’s why…

When you have surgery, your tissues are ‘injured’, albeit in a very controlled, sterile environment. But all the same, the tissue does undergo an injury. Our body responds to that with an “inflammatory response”. Now the term ‘inflammation’ has gotten a bad rap of late but in reality the inflammatory response is the normal pathway by which the body heals. Part of this response is that the blood vessels in the area of the surgery will get more ‘leaky’. Blood vessels are not solid tubes, they are porous to let the cells that promote healing access the area. With this movement of cells come shifts of fluid from inside the blood vessels to to injured tissue, thus the swelling. As the healing process progresses the swelling will subside.

In general, swelling in the face resolves more quickly while the breasts and body take longer. The thighs and legs will stay swollen the longest because of gravity. So, in the face it is not unusual or unexpected to have swelling for several weeks. The breasts and torso will show swelling for 3 to 6 months typically.

There are methods to minimize the swelling that your surgeon will include as part of your aftercare so check with him/her as you prepare for your procedure. But in the end, Yes it is normal to swell and it always goes away.

Protect your investment.

allysonI can not stress enough the importance of using SPF daily. Too many people think that they only need sunscreen when they are “going out in the sun”. All of those people don’t realize that the majority of sun exposure we get is incidental. When you walk out of your house, to your car, or any time (during the day) you are not wearing SPF,  sun damage is happening. Sun damage will occur in the form brown spots, broken blood vessels, wrinkles, fine lines, even skin cancer. If you want to look young for as long as possible, wear SPF daily.  If you are investing in Botox/Dysport , Filler, IPL, Chemical Peels, etc, you must protect your investment and wear sunscreen.  I am currently obsessed with SkinMedica Total Defense and Repair.  It is labeled a “Super Screen” because not only does it protect from UVA/UVB but also from infrared and heat.  It is the currently the only super screen on the market with this capability.  I also love the ZO Oclipse Sunscreen and primer.  It is a physical sunscreen, which means it does not penetrate, it sits on top of the skin.  It is a broad spectrum, SPF 30 that also behaves as a makeup primer.  (love it)

Plastic Surgery for Weight Loss

scaleOften times when I consult with patients who are considering a tummy tuck or liposuction, they will ask “How much weight will I lose?”. Unfortunately, for most patients the answer is a pound or two, so not very much. Plastic Surgery is just not intended as a weight loss tool.

While both procedures do remove fatty tissue, and in the case of a tummy tuck, loose skin, these just don’t weigh very much. Fat as a tissue, is not very dense. It takes a lot of fat to weigh even a pound. I always weigh the amount removed when I do a tummy tuck and it almost always between 2 to 3 pounds, that’s it. This if for the patients who have a fairly generous roll between their belly button and pubic area. For the patients who really only have loose skin, usually from babies, it might not even weigh a pound.

With lipo, the same applies. I am able to suction out the fat, but fat just doesn’t weigh that much. In the ‘pound of feathers vs pound of lead’ comparison, fat is in the feather category and a tissue like muscle is in the lead category.

What these operations are really intended to do is to shape and contour your figure by trimming off inches. They tighten loose skin and remove stubborn fat pockets to balance your body and improve your shape. They also change dress sizes for a lot of patients. So many patients tell me they have to buy larger sizes to accomodate their thighs or hips or tummy only to have their jeans or pants then be too loose every where else. These are the kinds of problems Lipo and Tummy Tucks do improve.

I have Saline Breast Implants…How do I know if they’re leaking?

breast implantSaline Breast Implants were the only implant available in the US from 1992 through 1996 for breast augmentation. In my practice, I get a lot of patients with questions about needing to replace or exchange these implants. First let me say that you DO NOT have to exchange your implants just because they are of a certain age. If they’re not broken, don’t fix them.

Now, sometimes they do fail. The failure rate at 10 years is about 2.5%, so it’s low but it does occur. So how do you know? Well, you’ll literally get a “flat tire”. But how this shows itself varies based on why the implant failed. There are 2 points of failure for a saline implant. The first is if the fill valve fails, the second is a shell failure. The fill valve is a one way trap door valve that we plug into to fill the implant. If the valve becomes incompetent, what you will see is that the upper half of the implant loses its volume. The valve is right in the middle of the implant so all the water above this will leak out. The bottom half will stay filled. So basically, your fullness above nipple goes away. If the second problem occurs, the outer shell gets a hole in it, the implant tends to deflate more  readily and the entire implant will empty. Your breast will return to its pre augmented size. It’s typically not hard to detect for obvious reason.

The leaked saline will not harm you. It is nothing but saline IV fluid, the very same stuff we give patients who need an IV. So there is no danger to it.

So that’s the story on saline implants and leaking. If for whatever reason you were worried your implants were leaking, just look in the mirror. If they look the same as always, they aren’t leaking. If they do leak, no need to panic, they are warrantied for your lifetime and an exchange is a quick and easy procedure.

Winter = Dry Skin

allysonWinter is coming…… Which means dry skin is coming with it. Even people with oily/oily in areas skin types will get dryer in the winter. This is a result from the combination of there being less moisture in the air and the heat being on which also drains the skin of moisture/hydration. What will greatly help with this dilemma is a Hyaluronic Acid serum added to your daily skincare regimen.
