A NEW generation of Sunscreen.

allysonI can not stress enough the importance of using SPF daily. Too many people think that they only need sunscreen when they are “going out in the sun”. All of those people don’t realize that the majority of sun exposure we get is incidental. When you walk out of your house, to your car, or any time (during the day) you are not wearing SPF,  sun damage is happening. Sun damage will occur in the form brown spots, broken blood vessels, wrinkles, fine lines, even skin cancer. If you want to look young for as long as possible, wear SPF daily.  If you are investing in Botox/Dysport , Filler, IPL, Chemical Peels, etc, you must protect your investment and wear sunscreen.  I am currently obsessed with SkinMedica Total Defense and Repair.  It is labeled a “Super Screen” because not only does it protect from UVA/UVB but also from infrared and heat.  It is the currently the only super screen on the market with this capability.  I also love the ZO Oclipse Sunscreen and primer.  It is a physical sunscreen, which means it does not penetrate, it sits on top of the skin.  It is a broad spectrum, SPF 30 that also behaves as a makeup primer.  (love it)

allysonWinter is coming…… Which means dry skin is coming with it. Even people with oily/oily in areas skin types will get dryer in the winter. This is a result from the combination of there being less moisture in the air and the heat being on which also drains the skin of moisture/hydration. What will greatly help with this dilemma is a Hyaluronic Acid serum added to your daily skincare regimen.

Hyaluronic Acid will not only help you hang on to the moisture you naturally produce but also what you are putting into your skin. Naturally found in the body, hyaluronic acid creates fullness, secures moisture, provides structural support to cells vital for collagen production, and can hold 1,000 times its weight in moisture!

What is cosmetic surgery? Well, it’s basically defined as a procedure or surgery that is intended to improve your appearance. Cosmetic procedures are divided into surgical vs non surgical. So let me break down the two categories and give examples and a brief description of the MediSpa procedures.

Botox/Dysport: These are two of the most common cosmetic treatments at this point. Both of these injectable treatments immobilize a muscle so that the skin above it can’t wrinkle. That means the frown lines between the eyes, the wrinkles on the forehead and your crows feet will disappear for 3-4 months. Repeat treatments are needed but well worth it.

Peels: Chemical peels are one of the mainstay of MediSpa skin care options. The peels utilize a light acid that exfoliates and tightens the skin.

Lasers/ IPL: Lasers and IPL use high intensity, focused light to treat a variety of skin conditions. Facial spider veins, excess skin pigment (brown spots), and hair are commonly treated. Almost everyone is a candidate for an IPL treatment. Stronger lasers also treat tattoo removal and deeper wrinkles.

Tattoo Removal: Tattoo removal is an in office laser procedure. Treatments do require local anesthesia but are well tolerated with no down time.

Fillers: Today the mainstay of fillers are Hyaluronic Acid fillers. HA is a protein that is already a part of your skin. Brand names you may have seen are Restylane, Juvederm and Perlane. As we age, the youthful fullness of our face is lost and these products are used to restore volume in the mid face, the area between the bottom of the eye and the top of the mouth. They usually last about 18 months and restore the youthful, full appearance to your face. Treatments are done in office and take about 20-30 minutes.

Cosmeceuticals: This is a term for prescription strength skin care products that are sold via Plastic Surgeons offices. These products contain prescription strength levels of fading creams for brown spots and pigment, glycolic and salicyclic/ acids to improve skin texture, Retin A for fine lines, growth factors for increased collagen production, acne treatments to prevent break outs, and improvements in redness, dullness and volume loss.

Please be careful when you buy beauty treatments on line.

Case in point. Today I saw a patient for an Internist friend of mine. The patient thought she had burned her face using a chemical peel kit a friend bought off the internet.

Chemical peels are great. The one she used was mainstream stuff and perfectly fine…but it was the perfectly wrong peel for her to use based on her skin type.

Prescription strength MediSpa treatments are potent products. Assuming you actually get what is advertised, these products need to be used by someone who has knowledge and experience! Otherwise, you very well may get a result you didn’t bargain for! Worse case scenario is the product is bogus and then who knows what you can end up with.

It’s your FACE! Spend a few dollars more and get your facials, botox, lasers etc…from someone you know and trust! It’s worth the extra.

No one feature says more about us than our eyes. So it’s no surprise that when we start to experience aging in this area people, and most importantly, you, notice it!

There are a few options that can help aging around the eyes.

If the problem is fine lines on the lower lids, and this is usually what people see first, skin care is the answer. A little Retin-A and some prescription strength skin care product and most women see a big improvement.

Botox is very helpful for Crow’s Feet. Two or 3 injections on the side of each eye can go a long way to smoothing out your crow’s feet. You will need to be re-treated every 3 or 4 months but it does work and the cost isn’t too high, around $120- $160 per treatment.

If you lower lids show bags and skin excess, you will be best benefited by surgery. The operation is called a blepharoplasty and is designed to get rid of your ‘bags’ and tighten up the skin.

Now, the upper lids can be a little more tricky because your brow, or forehead, comes into play. More often than not, in my experience with Louisville Blepharoplasty patients, I find that most have some component of brow droop and upper lid skin excess.

Ideally, most patients would have an upper lid bleph and a brow lift. The reality is most people don’t want their brow lifted. That’s ok and we just do their upper lids.

In summary, your options, as is true for most facial aging, start with skin care and end with surgery and there is a lot in between. You just need to pick the starting point you are comfortable with and get started!

This is a pretty common question I  hear in my office. Women will come in with come concerns about their facial aging and want to know their options.

After eleven years of doing Face Lifts in Louisville, I have come to realize that there are a lot of women who want to look as good as they can, without surgery. In response, I have come up with a treatment plan that you through your options from least to most.

The ‘least’ end of the spectrum is better skin care products. I have a blog on prescription strength skin care products and I recommend you take a look at it. In brief, whatever you are buying at the cosmetic counters can’t compare to the skin care you can get from your plastic surgeon. Why? Because, by law, we can sell you products that are much stronger and thus much more effective. And, some component, like Retin-A, aren’t available without a presciption. So step 1 is to start on some good skin care.

After skin care, you are looking at deeper chemical peels and lasers. Though the methods by which they work are different, they have a common endpoint. That being that they wound your skin and when it heals it is better than it was before. Now these treatments carry a bit more downside on the recovery arena, particularly with the lasers. I deep peel or laser treatment can put you out of commission for 3 to 5 days.

If you have tried all of the above, and still aren’t seeing the improvement you would like, fillers and botox are options. Fillers and botox are office treatments that are quick, easy, and very effective. They are not permanent options but the cost has gotten low enough that many can afford to re-treat when necessary.

Last but not least is surgery. On the face we offer brow lifts, face lifts, eyelid tucks (blepharoplasty), and rhinoplasty (nose reshaping). For some, surgery is just never going to be an option, and that’s fine. But, there are a lot of possibilities and significant improvements are achievable. Of course the cost and downtime are greater, but the flip side is you do get what you pay for.

You can’t have a mini procedure, with a mini cost and a mini recovery and expect a maximum result. Sounds good but like most things in life, it doesn’t work that way…except for botox. That stuff is magic!

Cosmeceuticals” are prescription strength skin care products. A few of the more popular ones are Obagi, Glytone, and Skin Medica. These products are sold exclusively from a physician’s office, usually a Plastic Surgeon or a Dermatologist. In many ways these products are similar to what you are able to purchase at any upscale cosmetics counter. But, there are some very real and important differences.

First, most effective skin care lines contain some form of acid, be it glycolic, trichloractetic etc… The advantage of the products sold at your doctor’s office is that they are stronger. Basically, we can offer more concentrated products which will give you a more noticeable result. For instance, the max concentration of glycolic acid is 2.5% in an over the counter product. The Cosmeceutical lines start at 5% and go up to 12.5%.

Some components of prescription lines are simply not available over the counter. Hydroquinone and Retin-A are examples. Hydroquinone is a fade cream that is commonly used to fade sun spots and help blend and smooth your complexion. Retin-A is a vitamin A derivative that is used on the fine lines, especially around the eyes. It promotes a gentle exfoliation of the skin and a firming and tightening.

So, while they are more expensive, prescription grade skin care product lines are definitely more effective. Basically, like many things in life, you get what you pay for.
